Athletes–Things to Know

All athletes must have up-to-date paperwork in order to practice/compete.  Completing paperwork is a family/guardian responsibility.

Athlete Paperwork:

For Athletes Renewing their Paperwork (READ ALL):

  • Athletes must have up-to-date paperwork or they will not be allowed to practice or compete.  Make note of expiration dates.
  • Renewal means that you will submit 2 pages including the application/information page signed by the parent/guardian and the medical/physical exam page, signed by the doctor.
  • Forms are stored electronically by SOMN and status and renewal dates are available through the SOMN roster via Wings leadership.
  • Leadership will notify you when a physical or consent is due.
  • The “application page” and the “physical” must be renewed every 3 years.
  • You may renew yearly if convenient.  This works for those who visit their doctor annually.
  • You MUST check all boxes YES or NO.  No blanks.  Read carefully.
  • You may have the clinician fill out  the physical page if one was done within one year. No need for a new appointment.
