Athletes–Things to Know

Athletes are the heart of our team and it is our goal to assist athletes meet their competition goals.  If athletes sign up for participation in a sport, we do expect regular attendance at practices.  We are turning interested athletes away from our team so do not sign up for sports unless the athlete is available to attend at least 80% of practices.

Athletes may practice and compete if all paperwork is up-to-date and on file with SOMN.  We will notify you when things are due.  Please follow up to make sure this responsibility of yours is met.

Communication:

The team uses gmail for communication and Google Forms for registration.  Please use our team email address for all team communication.  In other words, do not use personal email addresses for team communication. We try to use blind copy when sending mass emails to keep email addresses private.  We only share team related information and do not share non-related topics that might be of interest to you.  Use our facebook page to share other messages.

Athlete Paperwork:

For Athletes Renewing their Paperwork (READ ALL):

  • Athletes must have up-to-date paperwork or they will not be allowed to practice or compete.  Make note of expiration dates.  
  • Renewal means that you will submit 2 pages including the application/information page signed by the parent/guardian and the medical/physical exam page, signed by the doctor.
  • Leadership will notify you when a physical or consent is due.
  • The “application page” and the “physical exam” must be renewed every 3 years.
  • All  boxes must be checked YES or NO.  No blanks. 
  • Consents are established when the athlete begins with Special O and updated ONLY again at age 18 or unless guardianship changes.
  • Submit pages directly to SOMN.  Info is on the form. 
  • Scan forms and email to: athletepaperwork@somn.org
  • No photographs, please. 
  • Fax: 612.333.8782  (BEST OPTION)
  • USPS Mail (takes forever).  Use email or fax is possible.  If mailing, keep a copy.

The full application contains:

  • Application/Athlete information (guardian’s sign this one)
  • Medical Form (health provider signs this form)
  • Official Consent form (renewed at age 18)
  • Healthy Athletes consent form
  • Concussion Awareness & Safety Recognition Policy
